Open Enrollment provides you with an opportunity to:
- Enroll in the Long Term Disability Plan - open for enrollment, this year only
- Change your Medical Plan from an HMO to a PPO (or vise versa)
- Change your Dental Plan from an HMO to a PPO (or vise versa)
- Add yourself and eligible family members to plan(s) previously waived
- Opt out of coverage

To Make Changes Your Action Is Required: Please go to the Garnett & Powers website and follow the prompts and process for Open Enrollment changes. You will be able to make changes via the UC ‘At Your Service’ website if you and your family members have social security numbers. However you will still need to complete the ‘Premium Agreement/Opt Out’ Worksheet and submit it to your departmental coordinator.
Changes made during Open Erollment will be effective January 1, 2010.
ATTENTION POSTDOC COORDINATORS:
Please review all forms for accuracy (including enrollment and personal information). Forward original forms to the Central Insurance Desk: Box 0832, Phone 476-8093, Fax 476-4449. The Central Insurance Desk Rep will forward the forms to the Payroll Department for input into OLPPS.
The UC contributions paid towards health benefits for some Postdocs are taxable.
